You will play a vital role in helping an aging population maintain their independence and well-being within their homes and communities.
This is a per diem position supporting a small group of a 17 clients, with an estimated time commitment of 5-10 hours per week. Additional hours may become available as more clients join the program.
What You’ll Do: As a Per Diem Community Care Registered Nurse with Mainstay, you’ll be at the forefront of delivering top-tier support services to seniors who require support with their activities of daily living. Your responsibilities will include:
- Collaborating with the program manger and support staff to provide high-quality non-clinical care.
- Conducting in-depth assessments of potential participants and monitoring their health status and community care needs.
- Maintain and update multiple client charts for each monthly visit.
- Building meaningful relationships with clients and caregivers.
- Ensuring compliance with Mainstay’s policies, procedures, and industry regulations.
- Evaluating and refining personalized care services plans to enhance client well-being.
- Engaging in regular in-home visits and documenting your interactions and observations.

Qualifications

A Day in the Life of a Home Health Aide:
Each shift brings a new opportunity to positively impact someone’s life. On shift you may help a client with dressing, grooming, and medication reminders. You also might prepare a meal together, provide companionship, and assist with light housekeeping. Throughout your shift, you’ll engage clients in meaningful activities, track daily goals, and ensure their comfort and well-being.

We’re looking for a Home Health Aides to work 6–10 hours per week, with a consistent schedule from Sunday through Tuesday.

We are seeking a kind, thoughtful, caring and dependable candidates to work Part-time as a Home Health Aide (HHA). The position includes providing 1:1 care to residents in a permanent supportive housing complex to keep their housing stabilized. This position will be part of a growing collaboration with Father Bills & Mainspring in Quincy. Services will be provided in the client’s apartment. Clients will have varied degrees of independence and a variety of support needs, including aging in place and physical/mental health disabilities.
This position will average 12-15 hours per week, one weekend day is required.
Duties and Responsibilities
- Engage in compassionate and upbeat exchanges with patience to meet people where they are and honor their current needs.
- Perform direct care guided by the client’s care plan. These responsibilities focus heavily to support residents 1:1 with their activities of daily living. These can include basic hygiene, dressing, exercising, positioning, feeding, toileting, medication reminders, ambulation and cue reminders for these areas.
- Assist clients with laundry and light housekeeping.
- Prepare meals for/with clients.
- Provide companionship to the clients, doing something mindful and fun, recreational during daily visits.
- Write daily service logs, track daily goals of the care plan, record service delivery on the GPS system.
- Report any client health status changes to supervisor.
- Represent Mainstay to the community and outside stakeholders with integrity.
Qualifications
Education and Experience:
- High School Diploma or equivalent, Home Health Aide (HHA) certification preferred.
- 1-3 years of experience working with individuals with a variety of disabilities, preferably in a residential setting.
- CPR and First Aid certifications (or ability to pass within the first 30 days of employment).
Skills, Knowledge, and Abilities:
- The ideal candidate will have excellent team player and relationship building skills and will collaboratively work with clients, program staff, nurses and others who are involved with the client’s care.
- Basic computer skills, including recording notes, use electronic time and attendance software and receive and send electronic mail.
- With support and training from your supervisor, ability to learn the procedures and policies of the MassHealth Group Adult Care Program (GAFC) –no prior experience with this program is expected.
